Description

Peace Harbor Medical Center Physician Assistant - Primary Care, Home Health & Hospice, Palliative Care, and/or Nursing Home/Assisted Living facility experience preferred.

• Flexible 4 or 5 day work week
• 100% home visits and facility visits
• Call: phone 1:3
• Average 6 patients/day
• Works independently in a collaboration with a Palliative Care Provider
• Provides care plans
• Primary care experience with the ability to treat palliative care patients with acute or chronic health conditions
• Counsels and educates patients, their families and caregivers
