We are recruiting a board-certified Internal, Family or Geriatric Medicine physician to join a multi-disciplinary practice in the greater Phoenix area.

* Primarily elderly medically complex patient population
* Enjoy smaller panel sizes
* Actively participate in a collaborative Care Team with Health Coaches, Nurses, Nurse Practitioners and Behavioral Health Specialists
* Collaborative happy work environment geared towards patient-centered care
* Face-to-face office visits and non-visit based interactions (e.g. phone, email, groups, home visits, video chat)
