NURSE PRACTITIONER OPPORTUNITY IN SYRACUSE, NY

Excellent opportunity for a New York licensed Nurse Practitioner in Syracuse, NY. This opportunity is with a national healthcare organization that provides home visits to their patients who have chronic diseases. Great position for someone that is looking for autonomy and flexibility.

The position

4-5 patient visits per day
Completes proactive, in-home comprehensive visits for patients with chronic diseases
Works collaboratively with patients PCP
Monday-Friday
Telephone triage call coverage every 4-5 weeks with call compensation

Qualifications

Active, unencumbered NY NP license
National board certification in adult, family or geriatric specialties
Has an active DEA
Current BLS medical provider
Must have state driver's license and automobile insurance
2 years experience in care of adult, chronically ill patients preferred
Must be willing to travel in a 50-mile radius from primary residence
