PACE is recruiting full-time advanced practitioners to join their dedicated and growing team of providers.  Opportunity to work as part of an interdisciplinary team to facilitate the care of participants.  PACE is a unique program of coordinated care that offers elderly adults and their caregivers an alternative to nursing home placement.  The mission is to uniquely bridge health and social supports to preserve independence and dignity.

TruChart LIFE electronic health records.  Monday - Friday schedule.  Physician must participate in provider call schedule (phone only).  Team includes medical director and advanced practitioner.

PACE is a community partnership between Spectrum Health Lakeland, Caring Circle, and Region IV Area Agency on Aging.  Physician will have privileges at Lakeland Medical Center, St. Joseph.
