Family Medicine

Black River Memorial Hospital is recruiting for a Family Medicine Advanced Practice Provider for our new clinic, opening July 2020.  This exempt, full-time position is benefited and day shift.  Black River Healthcare Clinic will provide a holistic approach by integrating behavioral health with primary care.

Essential Duties:

Provide direct patient care within the scope of primary care medicine in an outpatient setting to all patients who seek care in the Black River Healthcare Clinic.

• Perform duties of a primary care provider in an outpatient medical clinic- examines, diagnoses and treats acute and chronic medical conditions.
• Develops treatment plans and manages patient health issues in accordance with established clinic protocols.
• Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury
• Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients' condition
• Follows all existing referral patterns with respect to the referral of patients for specialist care
• Uses currently accepted methods and practices of medicine, adheres to applicable standards of care, medical ethics, policies, and protocols.
• Provides care that reflects health care cost consciousness and addresses social determinants of health.
• Maintains accurate electronic health records of patients and completes documentation in a timely manner.

   

  Educational/Certification Qualifications:

  • Advanced Practice Degree as a Physician Assistant or Nurse Practitioner.
  • License to practice in the State of Wisconsin.
  • Board certification is required.
  • DEA license.
  • Basic Life Support certification.
