We are seeking a full-time Psychiatric Mental Health Nurse Practitioner to provide outpatient services for our site located in Altoona, PA. We are looking for an individual who believes in making a difference in their community while working alongside other passionate providers to provide outpatient comprehensive psychiatric care to patients from pediatrics through senior citizens.  Our nursing staff supports each prescribing professional with gathering all vitals, medication reconciliation, updates to all clinical lists including allergy, directives, diagnosis and other related data. This information is gathered each visit and is prior to you seeing the patient for their psychiatric evaluation or medication check.

Practice Information:  The candidate will work closely with (5) psychiatrists; (5) PMHNP; (8) LCSW; nursing, and support staff. We see approximately 3000 patient visits per month which include psychiatry and therapy. Our nursing staff supports each prescribing professional with gathering all vitals; medication reconciliation, updates to all clinical lists. Primary Health Network (PHN) is a Federally Qualified Health Center that is TJC approved. We are celebrating our 35th year of embracing excellence in healthcare. At Primary Health Network we strive to assist our patients toward a fuller, healthier lifestyle. PHN accepts all patients regardless of their insurance or financial status and offer the sliding fee scale to all income eligible uninsured or underinsured patients based on annual household income. PHN offers 340b drug pricing and a patient portal.

Requirements to include: Graduate from an accredited School for Nurse Practitioners; Certification as a Psychiatric Mental Health Nurse Practitioner; Current PA License; Current DEA License; Knowledge of Electronic Health Records; Ability to provide quality, comprehensive, continuity of medical care in a compassionate manner with minimum supervision.

Area Description– Altoona is located approximately 2.5 hours East of Pittsburgh and 3.5 hours Northwest of Philadelphia. The city has approximately 48,000 residents and offers a great deal of variety for living situations. State College is only 45 minutes to our East which offers a very diverse cultural experience for any family and is known for safety in the community.
